If you are also using APEX there are further considerations. e.g. to upgrade from 11.2.0.3 running APEX 4.2 to 19c running APEX 21 one option would be to first upgrade the database to 12.1.0.2 (the latest version to still support APEX 4.2 and a version that can be directly upgraded to 19c) then upgrade APEX to 21 (since this is supported on db 12.1.02 and 19c), then upgrade the db to 19c.
